What is Social Prescribing

Social Prescribing is about supporting the health and wellbeing of people, spending time with the person and linking them in with sources of support and activities within their communities.

It provides GPs and other primary care professionals with non-medical referral options to improve Health and Wellbeing.

How it Works

Your GP or another clinician will refer you to the Social Prescribing Link Worker if you are suitable for the programme or you can self-refer. Our social prescribing Link Worker will meet with you to discuss your needs and the various Social Prescribing options available. You will begin your chosen activity with support.
The Link Worker will keep in contact and meet with you after a number of sessions to discuss how you are getting on and your interest in continuing with your new activity such as exercise, art, reading volunteering and gardening.

Who is it for?

It’s for adults over 18 years feeling isolated, stressed, anxious, lonely, and depressed. who may feel they need some support to mind their health and wellbeing, The Social Prescribing Link worker will help you to benefit from meeting other people or taking part in a new activity using resources within the community?

The core principles of Social Prescribing are that it:

• Is a holistic approach focusing on individuals’ needs and preferences?
• Recognise that people’s health is determined primarily by a range of social, economic, and environmental factors.
• Promotes Health and well-being and reduces Health inequalities in a community setting, using non-clinical methods (although alongside medical intervention) addresses barriers to engagement and enables people to play an active part in their health and well-being.
• Uses and builds on the local community assets in developing and delivering the service or activity.
• Aims to increase people’s control over their health and lives (Public Health England, 2019).
